How we handle your personal and health information.
Aevum Health & Physiotherapy (ABN on request) is committed to protecting your privacy. We handle personal and health information in line with the Australian Privacy Act 1988, the Australian Privacy Principles and the NSW Health Records and Information Privacy Act 2002.
What we collect
When you book, attend or contact us we may collect your name, contact details, date of birth, Medicare, DVA, NDIS, private health fund or workers compensation details, your health history, and the clinical notes we make during your care. Our website callback forms collect your name, phone number and the message you send us.
How we use it
We use your information to provide and coordinate your care, to process claims and payments, to communicate with you about appointments, and, where you have agreed, to send you clinic news. Clinical information is shared with other health providers involved in your care, with your funding body where a claim requires it, and otherwise only with your consent or where the law requires it.
Online booking and website tools
Online bookings are made through Halaxy, whose own privacy policy applies to the information you enter there. Our website uses Google Analytics and the Meta pixel to understand how visitors use the site and to measure advertising; these tools set cookies and may collect your device and browsing information. You can block cookies in your browser settings.
Storage and security
Records are held in secure practice-management software and in locked clinic premises. Access is limited to staff who need it for your care or for administration. We keep health records for the period required by law, then destroy or de-identify them.
Access and correction
You can ask to see or correct the information we hold about you. Contact reception at Kirrawee on (02) 8544 3231 or email info@aevumhealth.com.au. If you have a complaint about how we have handled your information and we cannot resolve it, you can contact the Office of the Australian Information Commissioner.
